Elyvate Wellness provides education and coaching and is not a medical provider. If you choose to submit wellness and intake information, we use it only to provide the Services and support your experience. Please do not share sensitive medical information you do not want stored in our systems.

We use reasonable safeguards to protect your information. No method of transmission or storage is 100% secure, and we cannot guarantee absolute security.

We keep your information as long as needed to provide the Services, meet legal and accounting requirements, resolve disputes, and enforce agreements. We may delete or de-identify data when it is no longer needed.
